County officials said all 12 deaths involving cocaine and fentanyl occurred within the previous three months. Investigators were still working to identify the source of the supply when the warning was issued. Officials did not establish whether the fentanyl was added intentionally or introduced through cross-contamination. Either way, the result is the same for the person receiving it: what was sold as cocaine also contained a potent opioid. People who use stimulants may not expect to experience an opioid overdose and may be less likely to keep naloxone nearby. Testing one portion only provides information about the portion that was dissolved and tested. Thoroughly mixing the full amount before sampling can reduce some of this uncertainty, but it cannot guarantee that every part of a sample has the same composition. A negative fentanyl result does not rule out every fentanyl analog or other synthetic opioids. Reagent testing also cannot confirm purity, potency, or every substance present in a mixture. These 12 deaths represent one county during one three-month period. They do not tell us how common fentanyl is in every cocaine supply. They do show why assumptions based on appearance, source, or expected drug category are unreliable. Someone can intend to use a stimulant and still encounter an opioid.
